limp mode loss of power

limp mode loss of power
Hello all, was hoping i would not have to post again this soon after my last problem....Anyway, was driving down the m5 the other day, put my foot down and suddenly "urgent engine service required" light flashed up on dash and i could not get above 2500 rpm. (Limp home mode)
Pulled over turned ignition off and then back on and all was fine, light went off. Then pulled away slowly at first and car was ok, then put foot down again and limp home activated again with the same error message.
Took car to local garage and they read the fault codes. There was two
1) p0094 fuel system leak
2) p0101 mass air flow sensor
mechanic cleared the codes and i went for a drive. put my foot down and limp home activated again. error codes read and this time it was the p0094 code logged.
the chap at the garage seems to think that its definately the mass flow sensor that is the problem. Seems to run fine up to 3500 rpm, when you accelerate above this or go up a steep hill, car goes into limp home.
right so if you could answer the following i would be most thankful:
1) does it sound like the MAF sensor at fault
2) what else could the p0094 error code mean, thorough visual inspection for leaks carried out. none found.
3) had fitted a diesel tuning module, could this have damaged the ecu or caused the problem?? have taken this out now and all codes cleared after removal.
4) what else do you think this could be?? no smoke, car performs just fine all the way up to 3500 rpm above error kick in.
