Hi,
Two answers:
1) Yes, a poor PCV system can definitely affect gas mileage and will eventually allow internal pressure in thecrankcase to rise, which will give you a nightmare (oil leaks everywhere, blown oil seals, etc...). If it hasn't been serviced in the last 3-4 years or so, I would service it now. If you can service it yourself, great but if not, you could perhaps buy OEM parts on-line and find a shop that accepts parts from customers. Expect the labour to be a bit higher, as shops make profit by selling you parts as well. Insist on OEM parts whenever servicing your Volvo (unless you plan to get rid of it within a year or two), as most cars are sensitive to the quality of parts, and lately there are many inferior aftermarket parts floating around. 2) This one I am not 100% sure. But if the fuel pump is in poor condition, it would give you starting/drivability problems. Fuel pump itself wouldn't get clogged, but there is a fuel strainer at the inlet of the pump and it is possible that the strainer can get clogged. Also, have the fuel filter replaced at every 30,000 miles or so. Owner's manual calls for filter replacement at every 60,000 miles, but I personally feel it's way too stretched. I personally replace mine at every 30,000 miles or 3 years.
