Default Braking problem

My breaks work fine most times.However, at times the car seems to slide as the breaks don't hold. This started happening all of a sudden and I changed my pad and bled the brakes and it is still happening.Any ideas

slides as in locks up or slides as in the abs is kicking in?

Slides as in the car is slowing down but not as it should. Not in the way as the ABS kicking in

The car does not skid. The break pedal is "high" and does not go all the way down.

Does the brake pedal get really hard to push when this happens?
