940 B230FD codes 2-3-1, 2-2-1

Hi all,
I am new to this forum and I'd really appreciate your help chasing some gremlins here. We have a US 1995 Volvo 945 with the non-turbo 2.3L engine with exhaust gas recirculation system. The car has 234K miles on it and as far as I know has the original engine. We bought the car at 230K miles. We named her Helga.
The engine runs reasonably well (a bit irregular at idle, not always super keen to rev and respond to throttle inputs but totally drive-able) but we have had issues with the check engine light being on. In order of chronology:
1. When we first bought the car it would throw error code 1-1-3 after about 20 minutes of driving and while cruising on the highway. I could reset the CEL and it would stay away if I avoided cruising.
2. After some time and as the weather around Boston got hotter and we fixed a leak in the exhaust just upstream of the oxygen sensor (unsure of causality), the error codes changed to 2-3-1 and 2-2-1 (lean mixture). After I have reset the CEL, this error comes on as soon as I start the engine!
