Here are my codes. I'm interpreting this as my problems are caused by a bad MAF and front O2 sensor:

I am the one that had the "missing" CEL bulb. Background: The car was running poor and I cleaned the MAF and she ran fine, no codes. Then about a week later (approx 300miles) she started the having same symptoms, that got worse the next time I started her. I have not driven her since because of the lean readings.

 

Check for Vacuum leaks.

 

Think that's all it is? Where can I get a vacuum diagram?

 

There should be one on the hood.
You can also spray carb clean around all hoses with the car running. If the idle changes there is a leak.

 

*update* Car has just been pretty much sitting in my driveway. No vacuum leaks. Should I start with replacing the O2 sensor?

 

You might want to start with a MAF first.
