Does anyone have any insight to this problem ? I ran into a puddle and now when the fan turns on the rad shakes really bad then turns off the shakes stop

Yes I am missing a blade would that be the culprit?

 

If you've got a broken fan blade that'll do it. The fans on the car are electric rather than old style belt driven, so they come on only when needed. The broken blade causes the fan to be out of balance, which shakes the fan housing, which shakes the radiator, which shakes the front support... on and on.

An aftermarket replacement will cost you around $125. Dorman part number 620-903 is available from Amazon (prime shipping even) for $122.85. Expect a Volvo OEM replacement part to set you back a lot more.
