It usually means either timing jumped,Someone did the timing belt and has it off,Or the shutoff valve on the top of the cam cover isn't working correctly.

Tech,
I've replaced vvt pulley also, it was fine for couple of days and now i have CEL again and code is p0013 which reads : Exhaust Camshaft Position Actuator Circuit / Open (Bank 1). Is it not recognizing something or...? If it's the sludge problem what is your advice, how to clean it right? thanks!

Here I am. Nothing good to report. [:@]I did 2 flushes in last 4 days, 2 gunk additives, 10 quarts of synthetic oil, 2 oil filters. No changes, car still runs great and CEL is still on.
Tech, I took the camshaft reset valve apart just to see what is inside. How is this thing picking up a signal? Oil or steam from the engine? Or maybeposition of vvt pulley?

Tech, we took vvt pulley out again and replaced it with new one, now timing is way off, car doesnt start. I assume everything including cam has moved from it's marks because vvt was under load. Where is the best place to start now? Also, could we damage the engine by trying to start it when everything is off its marks?

Yes it if was off more than 3 teeth most likely you bent valves.
You will need to either buy or make a cam lock tool to hold the rear of the cams in place.
Then you will need to set the pulley back up and try and see how bad the damage is. The car might not even start after setting it all back up.
I wish I had known you were going to do that. When the belt comes off the VVT pulley is supposed to lock into position and not move freely unless the cam is spinning. If it doesn't lock right away you give it a slight tap to make it lock.
But also the cams are supposed to be locked before removing any of the pulleys anyway. I was trying to find a picture of the tool but cannot seem to find one.
Edit here is one the tool with the red arrow pointing to it is the cam lock tool.
