Default V40 rear brake pads fitting

How do I change the rear brake pads on my 1999 V40? Do I have to screw in the big round bit on the body of the caliper to make room for the new pads?

The piston has to be pressed in while turning. There is a tool for doing this that you can borrow from Auto Zone or buy from Harbor Freight. Astro Pneumatic make one.

Some folks have been able to do it with more common tools (needle nose pliers).
