Default Coolant in oil '99 V70XC -Oil Trap?

Apologies if this has been answered. Can't find much in the searches I've done. We have a '99 V70XC with approx. 160K mi. Has water in oil. I want to look into other possible causes before tearing into it and pulling the head. In searching, I've seen other people mention the PCV Oil Trap as a possible cause for water in the oil. If this is true, can anyone explain how this thing works, and how it could cause water in oil? Also heard mention of the oil cooler in the radiator. I was going to try and bypass this, but the only lines I see going to the radiator appear to be transmission cooler lines. Any thoughts? Or should I pull the head?
