Default Stall/loss of power from standstill

Have a 91 240 wagon. When applying the gas from a standstill, the car sometimes seems to stall as if it has lost all power, then after a few seconds will kick in and be fine. It is now also started to occur intermittently when accelerating while moving, but usually it's fine once the car gets going. I am most concerned that the stall will not stop, as it now does, and I do not know where to start looking. Thanks!

I chased this for ages on my 740 turbo, a hesitation off the line, then normal power and all is fine. Was completely random. No codes or any other indications.
A new O2 sensor radically reduced the problem, but a new coolant temp sensor fixed it completely. One of the more critical areas of driveability that really needs accurate fuel/air ratios is just off idle.
