Did you mean these? I have only seen separate versions but not the ones that are "combined".

You can find them under the black plastic cover that is hiding the throttle body area (there may be other areas that have them so look in the engine compartment real good), which is held by one Torx screw. I'll attach pics of new ones found at:

Most of the cars have one port capped off you can use this port to run through the seafoam. Let us know how that works.
