the lever which is pulled to open the door appears to have disengaged inside the door.

I see no way to get to it other than removing the entire door panel. I have to roll down the window and use outside handle to open.

So, how is the door panel removed?

The plastic clips that hold the panel on operate like a pop rivet. Gently slip a small screwdriver under each one and pry it up to remove it.

 

They come out even easier if you use a small screwdriver to pop the center of the clip in to release it.
