Default Jerkiness after startup / acceleration

Hi, I have a 2004 S60 T5 (manual), 50,000 miles (82,000kms).

Sometimes, but not all times after I start my car and drive off it is really jerky. Only at about 10-20 km/hr (1st / 2nd). It gets so bad that I push in the clutch and it stops. I then slowly release the cluth and drive off slowly

I replaced the lower trans mount and it still does this occasionally. I find that if I let the car warm up (in summer) for a minute that this problem rarely occurs. Also, after I have been driving for 5 minutes, all accelerating after that is fine.

Any ideas whats wrong?

Maybe pressure plate going bad.
Original clutch?

Yes it is the original clutch. Are these things covered under warranty?

If not, what do they normally cost?

It might be I would have them look at it first. They might say it is wear and tear but I would check to make sure.
