Default Power Stearing belt/ crank pulley

I have a 1986 Volvo 740 GLE with about 150,000 miles on it. i know she is getting middle aged and can be temperamental at time but lately she has been knocking off her power steering belt, which also goes to the motor fan. I have narrowed the problem to a loose crank pulley but every time i try and tighten it it seems to get worse. Has anyone else had this problem or have any idea as to what i can do? the crank pulley has been making a rattling sound for about the last 3 weeks and now appears to be loose, could these pulleys be mounted with reverse threads?

Maybe the bolt isn't loose.
Maybe it is the outer part of the pulley that came loose from the inner part.
