start, run and go
1990 240 DL
Might be more than one problem. Have changed plugs, wires, rotor, cap, air filter and o2 sensor.
All vacuum lines have been checked (twice).
Have to depress and/or pump the accelerator to start and keep running when cold.
When warm does not idle smooth.
Some of the time when on the road it starts to accelerate than hesitates, other times it goes till I lift my foot.

If it has a cold start injector it wouldn't kick in unless it was below freezing.
I would start by making sure the throttle body and idle air controller are clean. Spray them with throttle body cleaner. If they're gummed up you will likely have idle issues.
